We are here to support older adults and their carers through wellness, reablement and restorative care. We’re helping people stay active, independent and connected in their community.
Our services are delivered by a skilled, multidisciplinary team with a strong focus on dementia and Parkinson’s care, tailored to meet individual needs and goals.

At home, evidence-based therapies that support health, wellbeing, and independence. All services are delivered within a wellness and reablement framework, consistent with CHSP guidelines.
Physiotherapy
Strength, balance, mobility and pain management.
Psychology
Confidential support for stress, grief, life adjustment and carer wellbeing.
Counselling
Private, compassionate support through stress, grief, life changes and carer wellbeing.
Occupational therapy
Support for safety, independence and everyday life.
Dietetics
Nutrition, appetite support and chronic disease management.
Cognitive Stimulation Therapy (CST)
A small group program for people with mild to moderate dementia.
Motiview
A virtual cycling program supporting movement, memory and social connection.
Group Dementia Support
A therapeutic program for people with dementia and their carers.

We welcome GP and health professional referrals, provider referrals, self-referrals, and family enquiries. Once we receive your enquiry, our team will contact you directly to explain the next steps.
